8/20/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home to conduct individual session with Ms. Broadnax. Ms. Broadnax appeared to have a positive mood. Upon arriving Ms. Broadnax appeared to be feeling better today as she reported to having less anxiety and tension. Ms. Broadnax did not present any symptoms of SI or HI. Intervention: QMHPA begin session by inquiring about Ms. Broadnax's emotional well-being. QMHPA reviewed with Ms. Broadnax on expectations for session and addressing her anxieties. QMHPA explored calming techniques that would support her with calming skills such as (deep breathing, and relaxation). QMHPA modeled effective ways Ms. Broadnax could reduce negative thoughts and focusing on things that are good. QMHPA assisted the Ms. Broadnax by demonstrating a relaxation technique for Ms. Broadnax to utilize daily when feeling overwhelmed, and stressed as she reported to wanting to yell and scream. QMHPA helped Ms. Broadnax identify a list of her triggers and explored how she allows them to dictate her daily. Response: Ms. Broadnax was open to QMHPA's suggestions for today's session. Ms. Broadnax verbalized that she feels like just screaming and yelling sometimes as well as feeling that she wants to destroy things. Ms. Broadnax verbalized that she struggles with using calming techniques as she becomes so frustrated and her negative thinking becomes problematic. At times she reports that she takes out her frustrations on others that are surrounding her. Ms. Broadnax was able to again write a list of her triggers and causes. She shared that she allows herself to become overwhelmed by things that she has no control.
